Water-Only Fasting

Water-only fasting involves complete abstinence from all calories while consuming plain water freely. Unlike intermittent fasting, water fasts are typically extended (24–120+ hours) and produce profound metabolic shifts: near-complete glycogen depletion by 24h, ketosis by 48h, and deep autophagy activation by 72h+. Used in integrative oncology for metabolic resetting, immune system renewal, and as preparation for chemotherapy. Valter Longo's research suggests that prolonged fasting (essentially water-only) promotes stem cell-based immune regeneration following chemotherapy — essentially 'rebooting' the immune system.

Mechanism of Action

Extended water fasting produces sequential metabolic phases: (1) glycogen depletion and early ketosis (12–24h); (2) full ketosis and significant IGF-1/insulin reduction (24–48h); (3) deep autophagy, mTOR suppression, and AMPK activation (48–72h+); (4) stem cell activation and immune regeneration (72h+, studied in mouse models and early human trials). Cancer cells — metabolically inflexible and dependent on glucose — are increasingly stressed as glucose availability drops. Normal cells efficiently switch to fat and ketone metabolism. At 72h+, white blood cell counts drop then rebound more robustly after refeeding — a potential immune 'reset'.

Protocols & Dosing

24-Hour Water Fast

Most accessible entry point. Begin after dinner (e.g. 7pm), fast until 7pm the following day. Drink 2–3 litres of water. Rest and reduce activity. Break with light broth or fruit.

3-Day (72h) Water Fast

The most studied duration for immune regeneration effects. Days 1–2 are hardest (hunger, mild headache). By day 3, many report increased mental clarity (ketosis). Medical supervision recommended. Electrolyte supplementation (sodium, potassium, magnesium) advisable.

5–7 Day Extended Fast (Clinic Setting)

Requires medical supervision. Blood glucose and electrolytes monitored daily. Used in some integrative cancer clinics in conjunction with tumour imaging. Not a home protocol.

Cautions & Contraindications

  • REQUIRES medical supervision for fasts exceeding 48 hours
  • Electrolyte imbalance (sodium, potassium, magnesium) is the primary risk — supplement or monitor
  • Do not drive or operate machinery on days 2–3 — cognitive effects of ketosis
  • Refeeding syndrome is a serious risk when breaking long fasts — start with broth, then soft food over 24–48h
  • Absolute contraindications: underweight, eating disorder history, Type 1 diabetes, pregnancy, adrenal insufficiency, cardiac arrhythmia
  • Break fast with care: vegetable broth → diluted juice → soft fruit → cooked vegetables
