Molecular hydrogen (H₂) gas is the smallest and most penetrant antioxidant known — crossing all cell membranes including the blood-brain barrier. Unlike conventional antioxidants, H₂ selectively neutralises only the most cytotoxic free radicals (hydroxyl radical •OH and peroxynitrite ONOO⁻) without scavenging the 'beneficial' ROS used by immune cells. In cancer, H₂ reduces chemotherapy/radiation side effects, supports quality of life, and shows direct anti-cancer activity in several cancer cell lines.

Mechanism of Action

H₂ selectively reacts with •OH (hydroxyl radical — the most damaging ROS) and ONOO⁻ to form water, without affecting H₂O₂ and O₂⁻ which immune cells use to kill pathogens. This selective antioxidant property means H₂ reduces oxidative damage from treatment without compromising immune function. H₂ also activates Nrf2 (master antioxidant pathway), inhibits NF-κB, and has direct anti-tumour activity by modulating mitochondrial membrane potential.

Protocols & Dosing

H₂-Rich Water

Drink 1.5–2 L hydrogen-rich water (0.5–1 ppm H₂) daily. Generated by hydrogen water generators or H₂ tablets dissolved in water. Drink immediately after generation — H₂ dissipates within 20–30 min.

H₂ Gas Inhalation

Inhale 1–4% H₂ + 96% oxygen mixture (safe concentration) via nasal cannula for 60 min twice daily. H₂ inhalation generators available for home use. Japanese hospitals use during chemotherapy.

IV H₂ Saline

IV hydrogen-rich saline infusion at integrative oncology clinic. Used in Japanese phase I/II trials alongside chemotherapy — showing reduced side effects.

Cautions & Contraindications

  • H₂ gas is flammable at >4% concentration — never use open flame near H₂ inhalation setup
  • H₂ tablets vary widely in quality — check independent lab testing for actual H₂ concentration
  • Drink H₂ water immediately — H₂ dissipates rapidly from solution
  • Generally very safe — no known toxicity at therapeutic concentrations
  • High-flow H₂ inhalation devices require quality devices — low-quality units may produce contaminants
