Constipation

Constipation affects up to 50% of cancer patients and is often worsened by opioid pain medications, anti-nausea drugs (ondansetron), dehydration, inactivity, and dietary changes. It can cause significant discomfort, bloating, and if severe, bowel obstruction. Prevention is much easier than treatment.

Herbs & Supplements — Safety Information

Herbal information is for educational purposes. Many herbs interact with chemotherapy and other medications — consult your oncologist before use.

When to Seek Medical Help Immediately

  • No bowel movement for 3 or more days
  • Severe abdominal pain or bloating
  • Blood in stool

5 Natural Remedies

🍋

Warm Lemon Water (Morning Ritual)

Best for: Daily prevention, mild constipation

Traditional Use

A glass of warm water first thing in the morning activates the gastrocolic reflex — the natural urge to open the bowels after waking. Lemon adds mild acidity that stimulates bile production and peristalsis. This is one of the simplest and most consistently effective natural laxative approaches.

🧪 How to Prepare

Juice of ½ lemon in 1 cup of warm (not boiling) water. Optionally add 1 tsp of honey. Drink immediately upon waking, before any food or coffee.

⏰ When to Take

Every morning as a preventive routine. Allow 15–30 minutes before eating.

Ground Flaxseeds

Best for: Prevention and treatment of constipation, daily maintenance

Strong Evidence

Flaxseeds are one of the richest sources of both soluble fibre (which softens stools) and insoluble fibre (which adds bulk and speeds transit). They also contain omega-3 fatty acids and lignans with anti-inflammatory properties. Multiple studies show ground flaxseed is as effective as psyllium for relieving constipation.

🧪 How to Prepare

Grind whole flaxseeds freshly (a coffee grinder works well) or buy pre-ground flaxseed meal. Add 1–2 tablespoons to porridge, yoghurt, smoothies, or sprinkle on food. Can also mix into water and drink immediately.

⏰ When to Take

Daily with meals. Drink at least 2 glasses of water with each dose — fibre without fluid can worsen constipation.

Active compound: Soluble and Insoluble Fibre, Mucilage, Lignans

Prune Juice or Stewed Prunes

Best for: Mild to moderate constipation, opioid-adjacent constipation support

Strong Evidence

Prunes (dried plums) contain sorbitol — a natural sugar alcohol with osmotic laxative effects — and chlorogenic acids that stimulate intestinal muscle contractions. Multiple studies confirm prunes are more effective than psyllium fibre for constipation.

🧪 How to Prepare

Drink 4–8 oz (120–240ml) of prune juice in the morning. Alternatively, eat 5–8 stewed prunes: simmer dried prunes in water for 10 minutes until soft, eat warm. Both work within 6–12 hours.

⏰ When to Take

In the morning with or before breakfast. Effect typically occurs within 6–12 hours.

Active compound: Sorbitol, Chlorogenic Acids, Dietary Fibre

Magnesium Citrate

Best for: Constipation with dehydration or low magnesium, opioid-related constipation

Strong Evidence

Magnesium citrate works as an osmotic laxative — it draws water into the intestines, softening stools and stimulating bowel movements. Unlike harsh stimulant laxatives, it is gentle on the bowel and is often used in oncology settings for chemotherapy-related constipation.

🧪 How to Prepare

Available as powder or capsules. Standard dose: 200–400mg magnesium (as magnesium citrate) at bedtime with a full glass of water. Effect typically occurs within 6–8 hours.

⏰ When to Take

At bedtime for morning relief. Can also use magnesium-rich foods: leafy greens, nuts, seeds, dark chocolate.

Active compound: Magnesium Citrate

🌿

Psyllium Husk

Best for: Ongoing constipation management, daily maintenance fibre

Strong Evidence

Psyllium husk is the highest-fibre natural supplement available, forming a gel in the intestines that bulks and softens stools. It is one of the few natural laxatives with Level 1 evidence and is endorsed by gastroenterology guidelines worldwide.

🧪 How to Prepare

1 teaspoon (5g) of psyllium husk powder stirred into a full 8oz glass of water. Drink immediately before it gels. Then drink another full glass of plain water. Available as Metamucil and other branded products.

⏰ When to Take

Once or twice daily. Must drink with and after — psyllium without adequate water causes intestinal blockage.

Active compound: Ispaghula mucilage (soluble fibre)
