Clinical trials specifically in cancer patientsMBSR was developed by Dr. Jon Kabat-Zinn at UMass Medical School and is one of the most extensively researched mind-body interventions in cancer care. Multiple meta-analyses show it significantly reduces anxiety, depression, fatigue, and sleep disturbance in cancer patients, while improving quality of life and immune function. MBSR teaches patients to observe their experience with non-judgmental awareness fundamentally changing their relationship with pain, fear, and uncertainty. It is recommended in oncology guidelines in the USA, UK, and Canada.

How It Works
Regular mindfulness practice thickens the prefrontal cortex (associated with executive control and emotional regulation) and shrinks the amygdala (fear response centre). Reduces cortisol and inflammatory cytokines directly beneficial in cancer. Activates the parasympathetic nervous system, counteracting the chronic fight-or-flight state that cancer patients are often locked in. Improves NK cell activity and immune surveillance.
What a Session Looks Like
8-week structured programme: 2.5-hour group sessions weekly, plus one all-day silent retreat in week 6. Daily home practice of 45 minutes (body scan, sitting meditation, gentle yoga). Available in-person, online, and as app-guided programmes. The full 8-week programme has the most evidence; shorter adaptations (MBSR-C for cancer) also available.
Cautions & Considerations
- The intensive daily practice requirement (45 min/day) can be challenging during active treatment
- Some patients experience increased distress initially as they become more aware of difficult emotions
- Not suitable as a standalone intervention in acute suicidal crisis
- Retreat day can be physically demanding cancer patients should inform facilitators of physical limitations
- Online/app-based versions have less evidence than the full structured programme
- Teachers vary widely in quality look for trained MBSR instructors (UMass Centre for Mindfulness certified)
