Dignity Therapy

Dignity Therapy was developed by Dr. Harvey Max Chochinov at the University of Manitoba specifically for patients facing life-limiting illness. It is a brief, structured interview that guides patients through a life review recording their story, values, lessons, and messages they wish to leave for loved ones. The interview is transcribed and edited into a 'generativity document' (a legacy document) given to the patient and their family. Studies show it reduces distress, increases sense of meaning, dignity, and purpose, and significantly helps family members in bereavement.

How It Works

Engages the dying person's 'generativity' the human need to leave something meaningful behind for future generations. The act of narrating one's life story and articulating values restores a sense of agency and continuity of self that serious illness can destroy. The physical generativity document serves as a concrete legacy something of themselves the patient can give to family reducing the existential sense of annihilation.

What a Session Looks Like

Single extended interview (6090 minutes) conducted by a trained therapist, nurse, or physician. Interview is audio-recorded, transcribed, and edited. Patient reviews and approves the document. Final document given to patient and family. Can be completed bedside in palliative care settings. Requires a trained Dignity Therapist training available through the Dignity in Care initiative.

Cautions & Considerations

  • Most appropriate for patients facing advanced or terminal illness less applicable early in cancer journey
  • Emotionally intense patients revisit their life story in the context of dying
  • Requires a trained Dignity Therapist not all palliative care teams have training
  • The interview and editing process can take 12 weeks requires energy the patient may not always have
  • Not a substitute for other psychological support works best as part of a broader palliative care package
  • Family members should be prepared to receive the document may need their own support
